Found a bug.
Lazily I set some respawn points and they fell down in a crack between some boxes and the outside wall. One respawn I had actually put me outside the outer wall where the pipes are. Pretty interesting way of getting out of the map.

I just came here to post the exact same thing. The disc has zero scratches on it and has worked fine up until the other day when I started getting some random DRE. I tried to load it up today only to find out that it can't read the disc at all now. Random side-note: any one else have a small issue getting access to the new maps in Forge? Every time I boot up, I go into the Forge menu and the maps are not there. I hit B to back out, then re-enter Forge and the new maps are on the list. It's not a big deal, but I haven't seen anyone else mention it.

Try switching the Lobby to Local and see what you can do.

I've had stretches of 10x in a row where it worked, and just now, the same cannon, didn't work 20x in row. Maddening.
Just spent over an hour doing nothing but trying to get them to work consistently - to no avail. So that map is down the tubes. I wish the soft ceiling was about five feet higher - that's all it would take.
