Serious, canonical answer: Pretty much the same thing as Venom. He was still very active in conflict zones and took a direct part in several wars.

No, Big Boss was rendering comatose for a second time following the final battle in MG2: Solid Snake. Huey Emmerich honestly advocated for an armament inspection in an effort to paint MSF in the public light as a militant organization for world peace. Big Boss and "Kazuhira" Miller feared that it would lead to their private group falling under United Nations regulation. He wasn't aware at all that Skull Face and XOF were going to execute their assault prior to the real UN team reaching the site. Also, what's up with Paz? Doesn't the ending confirm that she blew up at the end of GZ? I checked the med bay assuming she's be gone now but she's still there. So she really did have a bomb in her vagina that was removed but not shown in GZ for some reason?
Venom Snake was still on Mother Base when listening to the truth tape from Big Boss based on the Diamond Dogs logo in the mirror reflection. A common interpretation concerns a time lapse occurring when he flips the cassette over to "Operation Intrude N313" which was the FOXHOUND mission for the Outer Heaven Uprising, the events of the original Metal Gear game. Once he punishes the mirror, it's notable that the O.H. logo has replaced the Diamond Dogs emblem. I believe the scene serves a symbolic purpose more than whether he's located at Mother Base or somewhere inland at the eventual site of his rest place. It probably wouldn't hurt to just believe that he's still running Diamond Dogs/Mother Base before inserting the tape next to the MSX2. There was absolutely nothing wrong with Skull Face, I foud that he was an excellent character. Kojima however did nothing special with him and he just died like a chump.

EDIT: I believe his betrayal was born out of his obsession to weapons of mass destruction. He willingly built D-Walker and Battle Gear as a display of loyalty, but he was indeed responsible for experimenting with the parasite outbreak on Mother Base which he intended to sell to Cipher.
Phantom Big Boss has a better marketing sense than Original Big Boss.

Her entire story-arc is tied to the extraction of wandering MSF soldiers in the side-ops list. Whenever you capture each respective trooper, you can just visit Paz to hand her a photograph which builds to a conclusion after ten separate trips. You're only about to witness a single scene per visit so you'll either have to wait twenty-four hours with the Phantom Cigar or depart/return from the ACC for the next opportunity to unlock. This game isn't canon so I wouldn't worry about names. Not to mention "Outer Heaven" is an idea, not necessarily a place. Besides, the real gangsta numba 1 triple OG Big Boss is the one that came up with the name Outer Heaven in PW.
